Notice of Limited On-site Hours Disclosure Form
It is required by law in the Commonwealth of Virginia that the owner of any animal left in the care of this facility be informed that the facility is not staffed on a 24-hour basis.
Whiskers & Tails Floyd Veterinary Hospital has business and medical staffing hours as follows:
Monday 8a – 6p
Tuesday 8a – 6p
Wednesday 8a – 6p
Thursday 8a – 6p
Friday 8a – 6p
Saturdays, Sundays, Holidays – CLOSED
THIS IS TO INFORM YOU THAT WE DO NOT HAVE CONTINUOUS STAFFING OR CARE:
1) OVERNIGHT: From closing time 6p (M, T, R) 1p (W) until opening 8a the following
business day
2) WEEKENDS: From closiong time 6p on Friday until opening 8a on Monday
3) HOLIDAYS: From closing time (6p) on the business day prior to the holiday or the Friday
before a Monday holiday until 8a on the next normally scheduled business day.
If 24-hour care is desired by you for you pet, or if it is recommended by our veterinarian as the best care or necessary treatment for your pet, the nearest 24-hour care facility is: Town and Country Veterinary Clinic and Emergency Hospital, 200 Central Ave, Christiansburg, VA 24073, (540-382-5042).
Written Prescription Policy
Some medications provided at Whiskers and Tails Floyd Veterinary Hospital may be available at an outside pharmacy. If an animal owner would like to have a prescription filled elsewhere, a written prescription may be requested. Additionally, some medications may not be available on site for purchase, in this case a written prescription may be provided.
- A written prescription requested or provided during the course of a veterinary appointment will be provided complimentary (no fee).
- A written prescription requested outside of a veterinary appointment (eg. by phone, e-mail, fax, drop by request, during another pet’s appointment) will incur a $5 fee for the doctor to review the patient file and issue the prescription.
- Once a written prescription is provided, it is the animal owner’s responsibility to provide the prescription to the pharmacist. Whiskers & Tails Floyd Veterinary Hospital will not be responsible for emailing, faxing, calling-in or otherwise sending the prescription to the client’s chosen pharmacy.
- Due to addictive nature and potential for abuse of certain medications, some prescriptions may not be written with refills authorized (eg. DEA controlled substances).
- Due to state law, controlled substances may only be dispensed from this hospital in a maximum of a 7 day supply. If a pet needs to be maintained on a controlled substance for a time period longer than 7 days, then a written prescription may be provided. It is the animal owner’s responsibility to fill that prescription at a pharmacy.
- Written prescriptions for controlled substances will not be replaced or reprinted if the animal owner reports the prescription lost or stolen, if the owner runs out of, loses, or misplaces the medication, or if the owner reports the medication as stolen. Due to state and federal law, a prescription for controlled substances may not be refilled early based on the prescribed usage. Written prescriptions for controlled substances will be for a maximum fill of 30 days. Dose changes by a physician, may require the owner to pick up a new prescription to be able to fill the prescription sooner than 1 month to accommodate the dose change.
Payment Agreement
Payment is due at the time of service for all outpatient visits. Deposits are required for hospitalized patients, including surgical patients. For in-patients (hospitalized or surgical), the balance is due at the time of discharge.
Forms of payment accepted include: cash, major credit card, debit card, personal check, and CareCredit. Post-dated checks will not be accepted. Checks returned for non-sufficient funds will incur a fee of $25 fee.
Payment plans are available through CareCredit and Cherry Financing. Financing is subject to credit approval. CareCredit allows for a 6 month payment plan. If payment is not completed in 6 months, interest is assessed. Cherry offers a soft credit check with multiple payment plans. Ask for more information if a payment plan is needed.
We will work with all Veterinary Insurance Providers. We do not bill the insurance provided. Policy holders will be responsible for paying their pets bill at Whiskers & Tails Floyd Veterinary Hospital and submitting records and forms to the insurance company for reimbursement. We can supply you with diagnosis information, doctor signatures, invoices and records as needed to obtain reimbursement of funds spent.
You will receive an estimate of charges prior to treatments or surgeries. Please understand that an estimate is a good faith best guess cost for procedures, however individual responses to treatments and medications vary and an estimate is not a quote or price guarantee.
Please understand that serious emergency conditions often require intensive testing and treatment, resulting in higher costs than the average, routine veterinary visit.
No show and cancellations: for all clients, if your appointment is missed, cancelled or rescheduled less than 24 hours in advance or if you are more than 10 minutes late for an appointment you will be placed on “pre-payment only” status. This means that you will have to pre-pay the exam fee at the time of scheduling. Pre-paid exam fees are NON-REFUNDABLE if they are missed, cancelled or rescheduled less or scheduled than 24 hours in advance.
